Please print out both the Registration & Parent Guideline forms (click below) and:

1. Fill out the Registration form
          (please print ....clearly).
2. Place the Parent Guidelines somewhere accessible
          (fridge door?)
3. Sign the registration form.
4. Tear off top portion & include cheque
